Confidentiality PolicyPRIVACY STATEMENT: The Gramm-Leach-Bliley Act was enacted to establish federal rules governing disclosure of personal information. We are delighted that this act of Congress now supports the privacy policies that have been practiced by our office since inception. We have always been bound by professional standards of confidentiality that are more stringent than those required by law. Therefore, we have always protected your right to privacy and take our relationship seriously. As a Podiatric Medical Surgical Professional Association we are now required to notify all our patients of our privacy policy. We are confident that you are aware that we respect the privacy and confidentiality of each and every patient. We collect and maintain the following information for each patient in our practice: 1. Information we receive from you on your encounter forms. 2. Information that we generate in the medical record. 3. Information about billing transactions that we send to your insurance company or companies 4. Information we receive as explanation of benefits from your insurance carrier. 5. Information requests from third parties, like your insurance company requiring more information to process your claim or from an attorney regarding disability or some other agency requesting information. We will not reveal patient information to any external source unless we have previously informed the patient by disclosure agreements, have been authorized by you the patient, or are required by law or by health insurance regulators. We maintain your records for professional services that we provide so that we are able to assist you with your professional needs. To safeguard your nonpublic personal information, we maintain physical, electronic and our own procedural safeguards that comply with our professional standards. Your confidence with us is important.
